Student Application Form for Safety Checking
under the Children’s Act 2014
The Children's Act (2014) requires education providers to complete safety checking of students before they begin field education, if it is likely that they could be working with a child without the presence of that child’s parent or guardian.
Before beginning field education in the social work programme at the University of Waikato students will complete a NZ police vetting check, a risk assessment questionnaire and provide the names and contact details of two nominated referees.
The information below outlines everything you will need to apply for admission to SOCWK300 and SOCWK400 - please read this information carefully.

1. Eligibility:
The prerequisites for SOCWK300 are:
HMDEV340 or HDCO340 ;
SOCWK302 or SOCW302 ;
SOCWK303 or SOCW303 ;
SOCWK306
- and all required papers in Years 1 and 2 of the BSW specified programme.
The prerequisites for SOCWK400 are:
SOCWK401 or SOCW401 or SOCWK490 or SOCW490 ;
SOCWK404 or SOCW404 ;
- and all required papers in Years 1, 2 and 3 of the BSW specified programme.
Admission into SOCWK300 and SOCWK400 is at the discretion of the Convenor of Social Work, and consideration of the police vetting check, risk assessment questionnaire and written referee reports will inform the decision.
2. The Application Process:
Once the prerequisites have been met students will complete the Student Application Form for Safety Checking under the Children's Act 2014 and the Children's Act 2014 Risk Assessment Questionnaire, and provide the names and contact details of two nominated referees.
Please be aware that referees must not be a family member, or people that you live with.
Suitable referees include current or recent employers, professionals who are familiar with your academic or volunteer activities. Please avoid family friends or people who know you only socially rather than in terms of your academic or professional work.
3. The Procedure We Follow:
Once the documentation listed above has been completed, and the referee checks have been received, the Lecturer - Social Work Field Education and Convenor of Social Work will review the documents.
If issues arise with the documentation, or the Police Vetting check the student will be directly contacted to discuss the issues.
4. Police Vetting
All applicants must undergo vetting by the New Zealand Police. Part of the vetting process is the need for students to hand in a physical copy of the New Zealand Police Vetting Form to either the Hamilton or Tauranga campus, along with (at least) two original identification documents, details of which are outlined in 5. (below).
If you have lived in New Zealand your whole life you will need to complete sections 2 and 3 of this 'Police Vetting Request and Consent Form (NZ Only)'
Should you have been a resident of Australia at any stage you will need to complete sections 2, 3 and 4 of this 'Police Vetting Request and Consent Form (NZ and Australia)'
If you have been a resident in a country other than New Zealand or Australia you will be required to provide a police certificate or equivalent documentation from that country, in addition to one of the above forms, whichever is appropriate.

You may chose when to do these forms. Either complete them now, in preparation for the application, or as part of the 'vetting' portion of this form, or after submitting this online application form. Please note, however, that you will need to have submitted this online form (or the equivalent paper application) before you can hand in your police vetting form or identification documents.
5. Acceptable Identity Documents
One of each type of document below is required. Originals documents must be presented. At least one identity documents must contain a photo.

- New Zealand emergency travel document
- New Zealand refugee travel document
- New Zealand certificate of identity (issued under the Passports Act 1992 to non-New Zealand citizens who cannot obtain a passport from their country of origin)
- New Zealand certificate of identity (issued under the Immigration Act 1987 to people who have refugee status)
- New Zealand firearms licence New Zealand Police
- New Zealand full birth certificate that is issued on or after 1 January 1998 and that carries a unique identification number Department of Internal Affairs
